  • For payment. To bill and be paid for the care you receive.
  • For health care operations. To run the platform, review quality, and verify that the people involved in your care are licensed.
  • Where the law requires it. Including reporting required by public health authorities and responding to lawful legal process.

Any other use or disclosure requires your written authorisation, which you can revoke at any time.

Your rights

  • To inspect and get a copy of your health information.
  • To ask us to correct information you believe is wrong.
  • To ask for an accounting of disclosures we have made.
