Your California privacy rights

Your rights under California privacy law, and the form for requesting a copy of the information we hold about you.

If you live in California, the California Consumer Privacy Act gives you specific rights over the personal information we hold about you. This page explains them and gives you the form to use.

Your rights

  • To know. What personal information we have collected about you, where it came from, why we have it and who we have shared it with.
  • To a copy. A portable copy of that information.
  • To delete. To ask us to delete it, subject to the records health law requires us to keep.
  • To correct. To have inaccurate information put right.
  • Not to be discriminated against for exercising any of these.

We do not sell your information

The Doc Connect does not sell personal information and does not share it for cross-context behavioural advertising. There is nothing to opt out of, because we do not do it.

Medical information is treated separately

Information held as part of your medical record is governed by health privacy law rather than by the CCPA. Your rights over it are in our Notice of Privacy Practices, and asking here will reach the same people.

Making a request

Use the form below. We will confirm we have it, and we will respond within 45 days. We may need to verify who you are before we hand anything over, which protects you rather than us. An authorised agent can make a request on your behalf with written permission.
